Is anyone onsite experiencing new or worsening symptoms of COVID-19?
Have you had any confirmed cases on of COVID-19 onsite within the last 14 days?
Do you have any unconfirmed or potential cases of COVID-19 from staff or contactors who have been onsite in the past 14 days?
If I become aware of any positive or potential COVID-19 cases for any employees or contractors onsite, I will contact G.C Duke Equipment prior to the technicians arrival to notifiy them, and understand this will result in my appointment being rescheduled.
